What Nursing Home Staffing Data Actually Tells You

Ask people who work in long-term care what single factor most shapes daily life in a nursing home, and staffing comes up before anything else. It determines how long a resident waits after pressing a call light, how well staff know each resident’s needs, and how much attention is available when something changes. The federal record contains unusually good data on it — if you know how to read it.

Where the numbers come from

Facilities report staffing through payroll-based journals — actual payroll records submitted to CMS, not self-described estimates. That makes staffing one of the harder-to-polish parts of the record. The data is also adjusted for how much care a facility’s residents actually need, so a building serving sicker residents isn’t penalized for needing more staff.

The two numbers worth knowing

• Registered-nurse hours per resident per day. RNs carry the clinical assessment responsibilities, so RN time is the sharpest indicator of clinical attention. Every ClearPath report shows this figure for every facility — and honestly marks it “Not reported” when a facility hasn’t filed it, which is itself worth noticing.
• Nursing staff turnover. High turnover means residents are repeatedly cared for by people still learning the building and their needs. Low turnover tends to travel with consistent care, familiar faces, and institutional memory.

How to read them together

Neither number means much in isolation. Strong hours with very high turnover suggests a facility keeping the schedule full with a revolving door. Modest hours with low turnover can deliver steadier care than the raw number implies. Questions the data should prompt on a visit

• How do you cover nights and weekends — and with your own staff or agency staff?
• What is your current nursing turnover, and what changed recently?
• Who assesses a resident when their condition changes overnight?
