Skilled Nursing Facility vs. Assisted Living — the quick version

Skilled Nursing Facilities (SNFs) provide 24-hour licensed nursing care. They are Medicare and Medicaid certified, federally inspected, and appear in the CMS national database. They’re appropriate when someone has complex medical needs, requires IV therapy, wound care, physical therapy, or is recovering from a hospital stay.

Assisted Living Communities help with daily activities — bathing, dressing, medication management — but do not provide 24-hour skilled nursing. They are licensed by individual states, not the federal government. They’re appropriate when someone needs personal care and supervision, but not intensive medical oversight.

We do not use self-reported data from facilities.
Facilities are not required to publicly disclose their monthly rates, and most don't. No independent source — including ours — has reliable pricing data for individual facilities. Your report includes regional cost benchmarks (Genworth Cost of Care Survey) so you can assess what care in your area typically costs, plus we identify which facilities are Medicaid-certified as an indicator of more accessible pricing. For exact rates, contact the facilities that appear highest in your results after reviewing your report.
Yes — in select states. ClearPath provides SNF (skilled nursing facility) reports for all 50 states using the federal CMS Care Compare database, which is updated monthly. For assisted living facilities (ALFs), ClearPath also generates reports in California, Maryland, Massachusetts, Michigan, Missouri, North Carolina, South Carolina, Virginia, Washington, and Wisconsin — states where official licensing data is published through verified, reliably updated government sources. If your state isn't on the assisted living facility list, you'll be notified during the intake process and offered a skilled nursing facility report instead. Coverage expands as additional states meet our data reliability standards.

If a new deficiency citation is added, a staffing rating changes, or a civil monetary penalty is issued, you'll receive an email alert with a plain-English explanation of what changed and what it means. This is particularly valuable for families whose loved one is already placed — most families have no way of knowing when a facility's record changes after move-in.

Decisions about senior care involve complex personal, medical, and financial factors that only you and your family — in consultation with appropriate professionals — can fully evaluate.
ClearPath sources assisted living facility data directly from each state's official licensing database — but only when that source meets our standards for accuracy, freshness, and ongoing reliability. To qualify, a source must provide machine-readable data through a stable government-maintained API or direct file download, require no special access or session-based authentication, and be verifiably updated on a regular schedule. Not every state publishes its ALF licensing data in a format that meets these standards. Some states use PDF documents that change format without notice, others rely on portal systems that are difficult to access programmatically, and others haven't yet modernized their data publishing infrastructure. Rather than pull data from unreliable sources and risk delivering inaccurate information to families making critical decisions, we don't include those states. ClearPath currently provides ALF reports for California, Maryland, Massachusetts, Michigan, Missouri, North Carolina, South Carolina, Virginia, Washington, and Wisconsin. As additional states update their reporting systems and meet our reliability standards, we will add them. If your state is not currently covered for ALF reports, ClearPath provides complete skilled nursing facility data for all 50 states — which may include the care options you need.
