This sets which CMS quality measures count most. Short-term rehab leans on short-stay results; long-term and hospice lean on long-stay results and staffing.
Does the person have dementia, Alzheimer’s, or memory problems that affect safety (for example, wandering or needing supervision)? *
A “yes” gives more weight to long-stay quality and staffing. CMS does not publish which nursing homes have a secured memory-care unit, so your report will include the questions to ask about that.

ClearPath pulls assisted living facility data directly from each state's official licensing database — but only when that source meets our standards for accuracy, freshness, and reliability. does not currently publish that data in a format we can verify and keep current, so we don't offer assisted living facility reports there. We'd rather be transparent than deliver something we can't stand behind.
If the person needs 24-hour nursing care, a nursing-home report may fit — ClearPath has nursing-home data for all 50 states, including . If not, we can’t help with assisted living in this state yet.
As states update their licensing systems and meet our reliability standards, ClearPath evaluates and adds them. Coverage will expand over time.
